Chiropractic for Low Back Pain

Summarized Review Conclusions

This research focused on the effectiveness of spinal manipulation combined with home exercise and advice for patients suffering from low back pain with associated leg pain (commonly referred to as sciatica). As someone who regularly treats patients dealing with sciatica and chronic low back pain, I found this study both clinically relevant and applicable. The research concluded that patients who received spinal manipulation and home exercise advice experienced greater reduction in pain and disability compared to those who only received exercise and advice. This is critical for patients who struggle with pain limiting their work, sleep, and daily activities. Understanding the value of combining chiropractic adjustments with proper home care is key for achieving long-term relief.

“Participants who received spinal manipulative therapy and home exercise with advice demonstrated significantly greater reduction in leg pain and improved disability scores compared to the exercise-only group.”
(Bronfort et al., 2014)

“The benefits of spinal manipulation were sustained through the 12-week follow-up, showing durable effects on both pain and function.”
(Bronfort et al., 2014)

“Patients who received spinal manipulation were more satisfied with their care compared to those who only received exercise and advice.”

Introduction to the Research

Low back pain with leg pain (sciatica) is one of the most common and debilitating complaints seen in chiropractic and medical clinics. The researchers aimed to determine if spinal manipulation could provide significant, lasting benefits when combined with home exercise and advice, compared to exercise alone. This matters because patients are often stuck between trying medications, injections, or surgery without understanding the potential power of conservative care. This study provides strong evidence that chiropractic care, when paired with active self-management, offers patients meaningful, lasting relief without the risks associated with more invasive procedures. Clinically, this research supports what we see every day — spinal manipulation is a powerful tool in reducing pain and improving quality of life.

Research Methodology

“The trial was a randomized controlled trial with adaptive allocation involving 192 patients suffering from subacute or chronic back-related leg pain. Patients were randomly assigned to either spinal manipulation plus home exercise with advice (SMT+HEA) or home exercise with advice only (HEA). Outcomes were assessed at 12 weeks and 52 weeks.”
(Bronfort et al., 2014)

Research Findings

“Participants in the SMT+HEA group experienced greater reductions in leg pain (primary outcome) and disability (secondary outcome) at 12 weeks compared to the HEA group. These improvements were sustained at 52 weeks for leg pain and satisfaction.”
(Bronfort et al., 2014)
